Establishing baseline status and separating pre-existing conditions from injury-related effects
Clarifying whether symptoms are ongoing, resolving, or likely to recur

Current diagnosis/diagnoses and supporting evidence
Symptoms, examination findings (if a clinical examination is instructed)
Functional impact: work, daily activities, continence/urinary symptoms where relevant
Current treatment and response
Future treatment needs and likely costs (where within remit)
Prognosis: expected recovery trajectory, long-term complications, and uncertainty ranges
Recommendations for further information or specialist input if required
